ABSTRACT

OBJECTIVE@#To observe the effect of nape cluster acupuncture on swallowing function and respiratory function in patients with post-stroke dysphagia, and to explore its relationship to cerebral arterial flow and neurotrophic factors.@*METHODS@#A total of 120 patients with post-stroke dysphagia were randomized into an observation group and a control group, 60 patients in each one. The conventional swallowing rehabilitation therapy and respiratory function training were adopted in the control group. On the basis of treatment in the control group, nape cluster acupuncture was applied at Fengchi (GB 20), Tianzhu (BL 10), Wangu (GB 12), Lianquan (CV 23), Panglianquan (Extra), once a day; pricking blood was applied at Jinjin (EX-HN 12) and Yuye (EX-HN 13), once every 2 days. Both groups were treated for 2 weeks. The therapeutic efficacy was compared between the two groups, and the swallowing function (scores of Kubota water swallowing test, standardized swallowing assessment [SSA] and video fluoroscopic swallowing study [VFSS]), the respiratory function indexes (forced vital capacity [FVC], maximal voluntary ventilation [MVV] and maximal expiratory time), the bilateral cerebral arterial hemodynamics parameters (systolic peak velocity [Vs], mean flow velocity [Vm] and vascular resistance index [RI]) and the serology indexes (brain-derived neurotrophic factor [BDNF], nerve growth factor [NGF] and insulin-like growth factors-1 [IGF-1]) before and after treatment were observed in the both groups.@*RESULTS@#The total effective rate was 80.0% (48/60) in the observation group, which was superior to 60.0% (36/60) in the control group (@*CONCLUSION@#On the basis of the conventional rehabilitation training, nape cluster acupuncture can effectively improve the swallowing function and respiratory function in patients with post-stroke dysphagia, its mechanism may be related to the improvement of cerebral hemodynamics and the regulation of neurotrophic factors.

ABSTRACT

Objective To investigate the blood pressure status of preschool children in urban areas of Qingdao, and to determine the relationship between obesity and blood pressure in preschool children. Methods A stratified cluster sampling method was used to select a total of 13 kindergartens in urban districts of Qingdao. Height, weight, waist circumference, hip circumference and blood pressure of children in three classes were measured. Body mass index (BMI), waist to hip ratio and waist to height ratio were calculated and the relationship between obesity and blood pressure was analyzed. Results The mean values of systolic and diastolic blood pressure in preschool children in urban areas of Qingdao were (95.52±7.66) and (62.78±6.52) mmHg, respectively. The detection rate of hypertension in preschool children was 13.50%. The SBP and DBP were positively correlated with BMI, waist circumference, hip circumference and waist to height ratio. There was a linear regression relationship between body mass index and age and blood pressure. The risk of hypertension in overweight and obese children was 5.191 and 2.824 times of normal body weight, respectively. Conclusions The prevalence of hypertension in preschool children in Qingdao urban areas is high.Overweight and obesity are risk factors for elevated blood pressure. Therefore, while preventing preschool children from obesity, preschool children's blood pressure monitoring and blood pressure monitoring and early intervention of hypertension of preschool children should be implemented.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the heterogeneity of systematic reviews (SRs) on nutrition support for burn patients with Meta-regression algorithm quantitatively.</p><p><b>METHODS</b>SRs on nutrition support for burn patients since 1980 were searched in 7 databases, including PubMed, EMBASE on CD, Science Citation Index, Cochrane Library, Chinese Biomedicine Database, Chinese Medical Current Contents, and Chinese Journal Full-text Database. SRs were identified with inclusion criteria: the research was interventive SR or Meta analysis of clinical data of burn patients aged equal to or over 18 years who had received clinical nutrition intervention, with regimen including special nutrient given including enterally or parenterally; the outcome indicator was mortality. A two-step method was established to evaluate the heterogeneity of SRs. (1) Qualitative analysis: the publication time of SRs and the trial design, patient type, regimen or route of nutrition therapy, energy intake, and outcome reported by the included clinical trial were analyzed. (2) Quantitative analysis: a mortality Meta-regression model was established by trial design, publication time, regimen or route of nutrition therapy, and energy intake to calculate the expected mortality. Observation/expectation (O/E) ratio and its 95% confidence interval were calculated to identify the heterogeneity among the clinical trials of SRs.</p><p><b>RESULTS</b>(1) Six SRs were retrieved which were published within 10 years. Only three SRs reported two of the three important parameters of nutrition intervention: regimen or route of nutrition therapy, energy intake, and clinical outcome. Totally, 11 clinical trials that gave outcome information were included in the SRs, including 10 RCTs and 1 case control trial. (2) Meta-regression analysis showed that significant heterogeneity existed in 4 RCTs (including five study groups), which were different from the other 7 trials. The patient samples of these 4 RCTs were from different population. No heterogeneity was detected by Meta-regression model among the three RCTs that reported regimen or route of nutrition therapy, energy intake, and outcome.</p><p><b>CONCLUSIONS</b>Quality of SRs on nutrition intervention for burn patients is poor, and significant heterogeneity exists among trials that are included in these SRs. O/E ratio from Meta-regression could be an effective tool to identify heterogeneity and its source.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate therapeutic effects of balance acupuncture on scapulohumeral periarthritis (SP)and the mechanisms.</p><p><b>METHODS</b>Thirty male New Zealand rabbits were randomly divided into a blank group (group B), a model group (group M) and a balance acupuncture group (group BA) with 10 rabbits in each group. SP model was established by using persistent mechanical strain and ice compress. Balance acupuncture at "Jiantong" point was applied in the animals in group BA, but not in the rabbits of the other two groups. Joint movement and pathological changes in tissues around the affected shoulder joint were observed, and interleukin-1beta (IL-1beta), 5-hydroxytryptamine (5-HT) in plasma and the affected surrounding tissues, and DNA contents in tendon of supraspinatus muscle were measured.</p><p><b>RESULTS</b>The content of plasma 5-HT in group BA [(18.16 +/- 4.44) ng/mL] was significantly lower than that in group M [(23.28 +/- 5.89) ng/mL] (P < 0.05), but without obvious difference in IL-1beta content between the two groups (P > 0.05). Compared with group M, the contents of IL-1beta, 5-HT in affected surrounding tissues, and DNA expression in tendon of supraspinatus muscle were significantly lowered to va rious extent in group BA (P < 0.01, P < 0.05).</p><p><b>CONCLUSION</b>Balance acupuncture can effectively improve local and systemic pathological situation induced by SP by way of reducing pain-producing factors, inflammatory factors and DNA expression to alleviate local organization and adhesions in rabbits with experimental SP.</p>

ABSTRACT

In combination with long-term clinical practice of acupuncture and moxibustion, and repeatedly learn, comprehend and probe theoretical intension about "essence" and "vitality" of acupuncture in Neijing (Internal Classic) to guide clinical practice. By the regulative mechanisms of needling "target points" of the peripheral nerve and central "target location", form single point therapy, rapid acupuncture, taking effect in three seconds, safety and other technique characteristics. Confirm that Deqi (needling sensation) is concrete reflection of "essence of acupuncture", regulating vitality (center) is the key of "first treating the vitality". The classical theory, "First treating vitality is essence for acupuncture" in Neijing, is correct and it is theoretical core of scientific acupuncture, which is of wide practical significance and profound historic significance for guiding clinical practice of acupuncture and moxibustion.

ABSTRACT

<p><b>OBJECTIVE</b>The purpose of the study was to explore a comprehensive management program for the obese children in kindergarten. The program should be scientific, rational, suitable for kindergarten, and easy to apply. Child care workers, parents, and child health care doctors participated in the program to help obese children establish a scientific life style through their daily life, to control their weight and to maintain their physical and mental health.</p><p><b>METHODS</b>The theories of behavior science, nutriology and kinematics were applied to make the weight control of obese children in kindergarten quantified and relatively standardized. Children were divided into three groups, intervened obese children, non-intervened obese children and normal children. Data such as weight, height, and body mass index (BMI) were observed to assess the effect of weight control and define whether it affects the linear gain of children's height.</p><p><b>RESULTS</b>The recovery rate of intervened obese group, non-intervened group was 62.5% and 5.3%, respectively. The general effective rate was 91.7% and 15.8%, respectively. The weight gain of intervened group was 4.15 kg lower than that of non-intervened group. The BMI of intervened group significantly decreased by 2.9 as compared to the non-intervened group. The height gain of intervened group was similar to that of normal group. Parents and health care workers' realization of children obesity and their corresponding actions, the behaviors of obese children at home and in kindergarten were changed toward the direction beneficial to weight control.</p><p><b>CONCLUSION</b>The implementation of the comprehensive intervention program for obesity in children, which involved obese children, child care worker, parents, and child care doctors alleviated the obesity level of those obese children without affecting their normal height gain. The program was safe and effective. Through the comprehensive intervention, the obese children could establish good habits during the sensitive period.</p>
